Home Delivered Meals

Homemaker service:
- light housekeeping
- provided for eligible home-bound seniors
Transportation supplied by the Connection
provides door to door transport to:
- nutrition site
- medical appointments
- essential shopping (groceries, prescriptions)
- Senior Center operations

information and referral services
- benefits counseling to assist with public benefits and insurance
- advocacy to assist with letter writing, appointments, and other objectives determined by participant
- health screenings for blood pressure, cholesterol, vision, hearing
- outreach to make target population aware of available services
- telephone reassurance provides daily or weekly contact with home bound participants by telephone
- recreation including games and presentations at the senior center
Case management
Assists participants in remaining independent at home.
Services include:
- assessment of need
- development of a plan of care to meet assessed needs
- monitoring of implemented services
Preventive health
- prescription drugs
- medical equipment
- treatment
- other health services not otherwise attainable by participants
Residential repair
- minor repairs or modifications to residence for safety/health reasons
- modifications for adjustment to new disability (wheelchair ramps, etc.)
- repairs or modifications not to exceed $700
- limited to one service annua
